Max caps used: Amox/AmoxClav 4,000 mg/day (1000 mg/dose); Cefdinir 600 mg/day (300 mg/dose); Cephalexin 4,000 mg/day (1000 mg/dose); Clindamycin 1,800 mg/day (600 mg/dose); TMP-SMX by TMP 320 mg/day (160 mg/dose); Azithromycin 500 mg day 1 then 250 mg/day (if 5-day regimen).
Quick Reference (typical outpatient ranges)
- Amoxicillin: 45 mg/kg/day divided q12h (or q8h) or 80–90 mg/kg/day q12h for AOM/sinusitis.
- Amox/Clav (ES preferred for high-dose): 45–90 mg/kg/day (amox) q12h. Dose by amoxicillin component.
- Cefdinir: 14 mg/kg/day total as 7 mg/kg q12h or 14 mg/kg q24h (max 600 mg/day).
- Cephalexin: 25–50 mg/kg/day divided q6–8h.
- Clindamycin: 20–40 mg/kg/day divided q6–8h.
- TMP-SMX: 8–12 mg/kg/day of TMP component divided q12h (susp: 40 mg TMP/5 mL).
- Azithromycin: 10 mg/kg day 1, then 5 mg/kg/day days 2–5 (some infections 10 mg/kg daily x3).
- Penicillin VK: 25–50 mg/kg/day divided q6–8h.
